Behavior bug (incorrect output/state without crash)

Model generated plausible but incorrect output due to insufficient grounding, missing verification, or high sampling temperature.

File-based memory (core-memory.md + daily logs) continues to work perfectly. Vector search is not blocking core functionality.

Please investigate batch result retrieval logic in memory indexing. The hang occurs after batch submission succeeds and batches complete on OpenAI’s side.
