Bug: HTTP 500 (server_error) from Codex does not trigger model fallback chain
증상
When the OpenAI Codex provider returns an HTTP 500 server_error, the model fallback chain does not trigger automatically. The user sees a raw error message in Discord and must manually switch models.

OpenClaw gateway, skill, or agent configuration issue — root cause confirmed in the openclaw/openclaw issue tracker.

- API 키 유효성/만료 확인
- OAuth 토큰 갱신: refresh token 사용
- 환경변수 확인: .env 파일 설정 검증
- 캐시된 인증 정보 삭제:
